Office Policies and Procedures
Confidentiality:
Information discussed during counseling sessions is
confidential and will not be released without a
client's written permission. Exceptions to
confidentiality under state law are:
1). Clear intent to harm oneself or others
2). Disclosure of physical or sexual abuse
3). A judge's written order to release information
pertaining to legal proceedings
Fees:
My fee is $90 per 50-minute session accepted at each
session. I accept insurance coverage for a limited
number of insurance companies. If you have
out-of-network coverage with your insurance company,
I am happy to provide written information for you to
file for partial reimbursement. I ask that clients
give 24 hours notice if cancellations need to be
made. Missed appointments are charged at regular
office rates.
Hours:
My office hours are generally Monday through Friday
from 9:00am - 6:00pm, however I can often adjust my
schedule. If you need an appointment outside of my
regular office hours please let me know.
